Redmine is creating duplicate issues in error

On rare occasions, probably about 5 times a year, Redmine creates the Project issue that we want to add, but then immediatly creates a duplicate issue.
The date and time of the issue creation is identical for both issues.

Both issues are identical in every way, including added files.

I have tried replicating the issue by trying to quickly double-click the Create issue button, but this only ever creates one issue, (as I want).
