Cannot add a document with File custom field

Hi,
I created File custom field, named it Document and marked as Required. After that I try to create an issue. I add a file, fill in all other required field, click OK but a message "Document cannot be blank" appears (please take a look at screenshots).
What else settings or permissions should I set?

The issue has been resolved. It appeared due to Usablity plug-in version 2.3.3. Now it has been fixed in Usability plug-in.
