how to add information form in front of "new issue"

Just in http://www.redmine.org/projects/redmine/issues/new, how can I add that information form (in yellow) ?

If you decide to hack the Redmine core in a rather hack-ish way, you can simply modify the view which belongs to the requested action (source:/trunk/app/views/issues/new.html.erb).
