404 Error when Creating New Issue/Attachments - Everything else works

Description

As mentioned, everything works, including: adding/editing users, creating projects, roles, modifying settings, etc.

However, when I try to create a New Issue, I get a 404 and the issue is NOT created in SQL Server. However, if I manually add an issue in SQL, I can view it in Redmine. Also, when I first go to "New Issue", it put a 0 in "parent task" and a "(NULL)" in the wysiwyg description of the issue.

Furthermore, whenever I try add an attachment, I also get the same problem.

I tried redeploying the .war, remigrating and population the sql tables and everything else I could think of. Still no resolution.

I'm using:
OS: Windows
Database: MS SQL Server 2008 R2
Web Server: Tomcat7
